This application form is for manufacturers exporting new vehicles and/or machinery from all countries to New Zealand and wish to have an MPI-Approved New Vehicle or Machinery System. This form also provides guidance around reducing the risk of biosecurity contamination within the vehicle or machinery supply chain.

The vessel master must complete this declaration form where it is intended a vessel will remain in NZ territorial waters (12 nautical miles) for an extended period of time and/or visit to any area of NZ that is not a Place of First Arrival for that vessel type.

Authorised under section 25 of the Biosecurity Act 1993 for the movement of uncleared risk goods that are herbaria specimens held in containment facilities approved to the MPI /EPA Standard for Containment Facilities for Plants: 2007 (155.04.09).

A detailed documented system is required showing that all Places of First Arrival (POFA) have the arrangements, facilities and systems in place to effectively manage risks associated with the importation of risk goods.

This application form is to be used for the purposes of applying for approval, (a) for a transitional facility &/or containment facility, under the Biosecurity Act 1993.
