This form is for sellers/distributors of hormonal growth promotants (HGPs) required to hold an MPI -approved operating plan under section 28 of the ACVM Act

Use this form when the only change to the wine standards management plan is a change of operator.

In special circumstances of death, bankruptcy, receivership or liquidation of an operator of a wine standards management plan, a new plan must be registered.

Use this form to request approval to import a product that is registered in NZ but will not be imported or sold with the approved NZ label.

Use this form if there a change in operator, or operator name, of a registered risk management programme.
