Use this form to notify deforestation of more than 2 hectares of pre-1990 forest land in each 5-year period commencing 1January 2008 under Climate Change Response Act 2002. Notification is required within 20 working days of the commencing deforestation.

Use this form when a Participant registered for a Post-1989 forest land activity:
- wishes to make a complete withdrawal (full exit) of all their Post-1989 forest land from the ETS, and have that activity removed from their entry in the Register of Participants;
- or where all their Post-1989 Forest land is no longer eligible.
Declaration that logs from the forest, block and compartment have been harvested from a forest that is surveyed and monitored under the Forest Owners Association Forest Health Surveillance Scheme.
Use this form to apply for recognition under the Wine Act 2003 to carry out wine verification activities and functions.
