This form may not be right for your type of complaint. Check this list before you start.
What type of eatery is it? Do not use this form if you're complaining about a food safety issue at a restaurant, eatery, café or takeaway (except from a fast-food chain). For complaints about those types of eateries, contact your local council and ask to speak to an environmental health officer or food safety officer.
