The information you provide on the Post Entry Quarantine (PEQ) Level 3B Form is used to help with our assessment and allocation of PEQ units in accordance with our Level 3B Greenhouse Booking and Prioritisation Policy and Part 3, section 16A of the Biosecurity Act 1993.

You are responsible for the accuracy of the information you provide, and any failure to complete all fields may result in your application being delayed or rejected.

All information provided to the Ministry for Primary Industries (MPI) is official information and may be subject to a request made under the Official Information Act 1982.
